Residential Case Coordinator

LIF Inc. is seeking a dedicated and organized Residential Case Coordinator to join our team, serving adolescent males ages 13-18 in our Kansas Department for Children and Families (DCF)-licensed YRC II facility. The Case Coordinator is responsible for developing, monitoring, and facilitating individualized service plans that support the safety, stability, and personal development of each youth in care.

Key Responsibilities:

  • Develop and maintain individualized treatment/service plans for each resident in accordance with DCF and YRC II standards.
  • Serve as the primary point of contact for youth case planning, collaborating with DCF workers, legal guardians, therapists, educational staff, probation officers, and other service providers.
  • Schedule and facilitate regular case plan reviews, team meetings, and staffings for each youth.
  • Ensure youth have access to appropriate educational, behavioral health, vocational, and medical services.
  • Complete and maintain accurate case files and documentation in a timely manner, including assessments, progress notes, incident reports, and discharge planning.
  • Support the implementation of trauma-informed care strategies and provide guidance to residential staff in addressing behavioral or emotional needs.
  • Participate in intake, orientation, and discharge processes.
  • Maintain compliance with state licensing regulations, agency policies, and accreditation standards.
  • Participate in crisis response and help de-escalate challenging behaviors in coordination with residential staff when needed.
  • Advocate for youth needs while promoting positive outcomes such as reunification, placement stability, or independent living preparation.
Qualifications:
  • Bachelor's degree in Social Work, Psychology, Human Services, or related field (Master's preferred).
  • Minimum 1-2 years of experience working with youth in residential care, foster care, juvenile justice, or related human services field.
  • Understanding of DCF policies, trauma-informed care principles, and adolescent development.
  • Strong communication, organizational, and documentation skills.
  • Ability to work in a collaborative, team-based environment.
  • Valid Kansas driver's license and reliable transportation.
  • Must pass KBI, DCF Child Abuse/Neglect Registry, and background checks.
