Job Objective: Work with individuals and families in a Transitional Housing/Rapid Rehousing Program on three basic outcomes: Daily Living, Financial Stability and Permanent Housing.Essential Functions: Conduct an initial assessment/history of the participantPrepare a case history, and develop, with the participant, a housing plan designed to end their homelessnessAssist the participant in the development and implementation of a housing plan and monitor/document their progress toward the completion of the planProvide linkages/referrals/transportation to appropriate agencies and function as an advocate for the participant in the community as appropriateProvide Case Management in participant's home, in the community and at the facility.Complete daily, weekly, monthly paperwork (Case Management meeting numbers, documentation, financial reports, check requests, beginning/end information in HMIS, etc.) in a timely and accurate mannerParticipate in the development of a discharge plan to facilitate the participants' obtaining/maintaining permanent housingGather statistical information as required by funding bodiesAttend community networking meetings to enhance the service delivery system for the clienteleProvide community education regarding homelessness and The Salvation Army by conducting tours, participating in workshops/in services, speaking to community groups, etc.Participate in agency and/or professional workshops/seminars to increase professional knowledgeRepresent the agency/program in the community as assignedProvide On-Call coverage as scheduledAs part of The Salvation Army comply with all confidentiality laws that apply to social welfareOther duties as assigned by the DirectorMinimum Qualifications: Education:Bachelor degree in Social Work or related fieldExperience:Prior experience in Social Services or residential setting; knowledge of homelessness and ability to work with and supervise a diverse clientele; knowledge of counseling/crisis intervention techniquesCertifications/Licenses:None RequiredSkills/Abilities:Effective communication skills (written and oral)Ability to prioritize job responsibilitiesKnowledge of community resourcesAbility to networkSupportive of administrative and program objectivesMust have a valid driver's license, maintain insurance, meet and maintain "qualified driver" statusAbility to operate basic office equipment including telephone, personal computerSupervisory Responsibility: Case Manager will supervise Intake Workers while on-callPhysical Requirements: Speaking, hearing and vision ability; manual dexterity; walking; sitting and standing for extended periods of time; lifting, pulling and pushing materials up to 25 pounds; Reasonable accommodation may be made to enable individuals to perform the essential functions of this job.Travel: May be required to attend conferencesDriving: Must possess a valid driver's license issued by the state of residence.
